United Healthcare Medicare Advantage Plan (UHC MA) has partnered with Physician Health Partners (PHP), an Accountable Care Organization (ACO).
Patients: If you are requesting a referral from your Piñon provider, please note that referrals can only be initiated for concerns that have been previously evaluated by our providers. Medical documentation and necessity must be substantiated, as required by insurance carriers and specialty care providers.
Specialists and Facilities: Our referral or order will be followed by a separate fax containing relevant clinical notes, labs, imaging, or other pertinent reports within 2–3 business days. If sufficient time has passed and you have not received the required documentation, or if it is needed more urgently, please contact our referrals line at extension 110 and leave a detailed message including your fax number.
United Healthcare Medicare Advantage plan patients and specialists: Due to high volume, these can take up to 7-10 business days for processing. However, as noted below - UHC/PHP is providing a grace period through April 2026.
